Page 43 – Cardiac Rehabilitation coverage – Standard Plan only – needs pre-authorization and limited to 24 sessions.
Page 44 – Specifically states Pulmonary rehabilitation not covered and Cardiac Rehabilitation not covered for Value Plan.
